SMS MESSAGING We may sometimes inform you about your test results / diagnosis via text message. By giving the surgery your mobile number consent to receive information via text message is implied. If you do not wish the surgery to contact you or via text please inform reception. 

LATE FOR AN APPOINTMENT - if you are late for the start of your appointment the clinican may not be able to see you. This is to reduce other patients wait time by sticking to their appointment times.

FLU VACCINATIONS In order to provide greater protection to our patients we are offering the QUADRIVALENT flu vaccination. Which protects against 4 strains of the INFLUENZA VIRUS.

Flu vaccination  If you did not take advantage of our Flu Vaccination drop in clinic you can still receive your flu vaccination. Contact reception to book an appointment

CHILDHOOD FLU VACCINATIONS. All children born between 1st September 2013 and 31st August 2015, or eligible patients who are under 18's that have a chronic disease can also have the nasal flu vaccination at the surgery. Please note if the child missed their flu vaccination at school please contact the school nurses to rebook it. 

PRESCRIPTION REQUESTS

Please click here for more information on how to order your repeat prescription. PLEASE NOTE THAT YOU ARE NO LONGER ABLE TO EMAIL PRESCRIPTION REQUESTS. This is due to the fact that the email address is not secure and should not be used for patient sensitive information.

Emergency Treatment after RTA's.

If you are seen at the surgery for emergency treatment after a road traffic accident you will be required to pay a fee of £21.30 which can be claimed back under the drivers insurance, see the main page for Further Information 

YELLOW FEVER CENTRE

We are now a Yellow Fever Centre and can offer Yellow Fever vaccinations to anyone who requires this.  You do not have to be a patient here to receive a Yellow Fever vaccination. Please contact the surgery to arrange an appointment with the nurse. The cost is £59.59 - this is for the vaccination and the certificate, payable in cash only at reception

Woodview Medical Centre welcomes new patients who reside permanently within the practice area boundaries.

Please use the web tool provided here to check if your postcode falls within our practice boundary.

Alternatively you can ask a receptionist.

British Citizens born in this country:

For each applicant applying to register you will need to provide the following:

  • Proof of identity (e.g. Valid passport, photo driving licence or birth certificate (or for new babies their 'red book')) 
  • Proof of address (e.g. Recent utility bill or bank statement less than three months old or official letter such as a mortgage or tenancy agreement)
     
Foreign Nationals not born in this country:

For each applicant applying to register you will need to provide the following:

  • Valid passport and visa
  • Proof of address  (e.g. Recent utility bill, at least three months recent bank statements or official letter such as a mortgage or tenancy agreement)
Please note : If you are not from an EU country you will need to have lived in the UK for 3 months otherwise you will be treated as a private patient until you have fulfilled the residency requirements.

Once the above criteria has been satisfied you will be given a registration pack with forms to complete. If you are on any regular medication you will need to make an appointment with a doctor and bring your medication to this appointment,
 

You will need to complete the following forms:

  • A GMS1 Form (or your current medical card)
  • A completed New-Patient Application Form
  • An Alcohol Screening Test
  • A Declaration Form
  • An immunisation history form (only for all children aged 5 and under)

  • New patient registration time Monday to Friday 12:45 - 13:45 then 17:00 - 18:00. Every second Saturday of the month 09:00 - 11:30.

  • Please note New registrations will not be accepted out of these hours. 

For your convenience these forms can be printed by clicking on the appropriate icon or you can pick up a registration pack from reception.

Please ensure you bring a complete registration and GSM1 form, Proof of photo ID and Address to the surgery. The patient must be present at time of registration.
