Admin Requests via Online Form

Dear patients, please be advised that with effect from 1^st December 2024, you will no longer be able to request prescriptions, test results, sick notes, or letters over the phone or by email. These requests must be submitted via the Online Form on the Admin Request page.

This change will help streamline our processes, ensuring requests are handled more efficiently.

You can also use the Airmid App to request prescriptions, view test results, sick notes and manage your health records easily. The app is fully integrated with our clinical system, providing quick and convenient access to your healthcare needs.
