Neighbourhood GP Practices
This illustrates the move towards a ‘Neighbourhood NHS’ Model. It will involve Primary Care Networks (PCNs) and Integrated Neighbourhood Teams (INTs) bringing together the following groups:
- GPs
- Nurses
- Social Care Staff
- Voluntary Sector Workers
The aim is to provide a joined-up, preventative and holistic care at Local level. They will expect to
Improve Access, Prevent Illness and Co-ordinate Care for Long-term conditions.
These groups do not correlate to the PCN (Primary Care Network)
Pilot Practices were requested for which Woodview stepped forward.
The Primary Care Network we belong to is called MWeb and consists of the following Practices:
- Woodview Medical Centre
- Moulton Surgery
- Kingsthorpe Medical Centre
- Denton Village Surgery
- Wootton Medical Centre
As a Group, we share the following Services:
- Physiotherapists
- Social Prescribers
- Clinical Pharmacists
- Pharmacy Technicians
- Safeguarding Co-Ordinator
- Trainee Nurse Associates
Awards
SM is very pleased to share that Woodview has won an Award for its work with Learning Disability Patients.
This is a huge credit to BJ and our new Nurse BS and RI Compliance Manager, who have devoted a lot time to working on the project.
RS brings great experience to the role and she will lead the Annual Learning Disability Reviews as well as the Severe Mental Health Physical Reviews.
Learning Disability Patients often find it very difficult to come into the Practice and the Practice is keen to make it as easy as possible for them.
