How to register as a new patient online

We are using a new online service called Register with a GP Surgery. That makes it easy to register with Woodview Medical.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

(Paper forms are still available if you need one)

Finding your NHS number

Click this link to get your NHS number.

Your NHS number is a 10 digit number, like 485 777 3456.

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Catchment area

 

Check you are in our catchment area.

Find out which GPs you can register with on the NHS website.

 

Change of address

If you are moving, please let us know as soon as possible. If you’re relocating outside the surgery’s catchment area, you may need to register with a surgery closer to your new home.

To update your address, telephone number, or email, we highly recommend using the Airmid or NHS apps for a quick and easy process. [Click here to download the Airmid app and NHS app].

Paper registration forms

 

If you prefer to register using a paper form, please come to our surgery reception to complete paper registration form as part of our registration procedure.

Registering as a temporary patient

You can register as a temporary resident with a GP surgery for up to 3 months. This can be helpful if you're living away from home but do not want to change your home GP surgery.

To register you'll need to fill out a temporary services form (GMS3), available from the surgery with your proof of address and photo of ID to complete paper registration form as part of our registration procedure.

After 3 months, you'll have to reapply to register as a temporary resident or become a permanent patient.

You may also be able to register as a temporary resident if you are visiting from abroad.

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient if you’re first language is not English, or if you’re an asylum seeker. You can find out more information about the NHS and how to register and access emergency services on the NHS website.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you.

To requests proxy access, please collect a proxy access form from reception from 8am to 6:30pm.

Linked profiles in your NHS account

 Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
